On September 4, 2026 Novartis announced that pelacarsen failed to meet its primary endpoint in the Phase III Lp(a)HORIZON trial, a result that challenges the idea that lowering lipoprotein(a) will automatically cut cardiovascular events. Although the topline result is disappointing, it will be scrutinized closely because it is the first definitive cardiovascular outcomes test of targeted Lp(a) lowering; researchers and drug developers must now examine subgroup data, event rates and the magnitude of Lp(a) reduction before drawing broad conclusions. If future analyses were to show benefit in a defined subgroup or with different dosing, the field could pivot rather than abandon the approach; nevertheless, the HORIZON outcome will reshape plans for new trials and for how clinicians think about treating inherited lipid risk.
