James Randolph Urbaniak was born on May 15, 1936, in Fairmont, West Virginia, and he died on July 18, 2026, at age 90. He was the Virginia Flowers Baker Professor and Chief of Orthopaedic Surgery at Duke University in Durham, North Carolina, and he served as director of the hand surgery program for decades.

Early in his career he turned his attention to the tiny blood vessels and tendons of the hand; he studied the circulation of the flexor pollicis longus tendon and became an early adopter of the operating microscope. He established Duke’s replantation service in the early 1970s, and his team performed complex replantations and toe-to-thumb transfers that expanded reconstructive options after severe trauma. Nevertheless, many challenging cases remained, and he worked to refine techniques and improve outcomes.

He contributed over 300 scientific articles and authored or edited textbooks in hand and microsurgery; he trained 328 residents, 124 hand fellows, and 81 international research fellows from more than 20 countries. His publications combined clinical series, technical descriptions, and basic research on blood flow after replantation. He served in leadership roles in national and international hand surgery organizations and received several awards for his work.

Urbaniak also helped develop a practical grading system for ring avulsion injuries, which is widely used in clinical decision making and is known as the Urbaniak classification. Moreover, his combination of clinical work, research, and teaching advanced microsurgery in orthopaedics and influenced how surgeons approach traumatic hand injury today. If future surgeons continue to use microsurgical replantation, they will build on techniques he helped develop.
